Submitting Academic Accommodations
Create an AIM profile using your Scripps’ email address.
Documentation must be provided on letterhead stationery, by a licensed or otherwise qualified professional who has undergone appropriate training and has no relationship with the individual being assessed. Documentation provided by a family member, even when otherwise qualified, will not be accepted as appropriate documentation.
Appropriate documentation must be provided by the treating health care professional on letterhead and must address the following guidelines:
- Licensure and experience of health care professional
- A statement of specific diagnosis and its limitations. Including diagnostic codes set forth by the Diagnostic Statistical Manual of the American Psychiatric Association (DSM-5) or International Classification of Diseases (ICD)
- A description of how the diagnosis was reached methods and procedures, test results and evaluation of test results
- What and how any major life activities are limited by the impairment
- How disability causes any impairment in the participation of the academic program
A list of any and all recommendations for accommodations which is to be accompanied by a clear rationale for recommendation. A recommendation does not necessitate automatic approval of said accommodations.
Upon completing an AIM profile and uploading supporting documentation, you should receive a confirmation email with a Calendly link in which you can schedule an intake appointment.
Students who experience a temporary physical or mobility impairment due to an injury or surgery must provide documentation of the impairment from a qualified examiner, such as a medical doctor or emergency room personnel.
- Reports should be on letterhead, typed, and legible.
- The name, title, and professional credentials of the physician and date of evaluation.
- Nature of the impairment.
- The expected duration of the impairment.
- The impact it has on the student. Please note, for lower limb injuries, the documentation must state that the student is non-weight bearing to be eligible for assistance with golf cart transportation.
- Suggested accommodations based on the impairment that is supported by the evaluation (i.e., golf cart transportation, scribing, note-taking, etc.)
